This product can be obtained through Affinity Financial Planner Great Eastern Life Indonesia 

General requirements:

• Participant Entry Age: 
• Insured: 14 days - 60 years
• Policy Holder: 18 - 80 years old
• Insurance Period: Until the Insured is 80 years old 
• Sum insured: 
  • Minimum Rp. 250 million 
  • Maximum: IDR 5 billion per insured (IDR 2 billion for child insured)
•  Prize: 
  • Single 
  • Regular premium for 5 years: Premium amount based on gender, sum insured and age
• Currency: Rupiah 
• Waiting Period: 90 days from the effective date of the Policy. Waiting Period applies to Critical Illness Benefits and verification of diagnosis confirmation.
• Costs: The premiums set by Great Eastern Life Indonesia include applicable costs, including marketing staff commissions and other commissions (if any). 

GREAT Critical Extra is a health insurance product issued by PT Great Eastern Life Indonesia. GREAT Critical Extra provides protection for up to 4 claims if diagnosed with different Critical Illnesses until the Insured reaches 80 years of age. This product provides Critical Illness Benefits, Premium Waiver Benefits, Diagnosis Confirmation Verification, Death Benefits, and Final Insurance Benefits.

This product offers both single and recurring premiums. Age is calculated using the most recent birthday method.
