* Disability Income Claim includes PayCare, PaySecure, and Pay Assure.
* Critical Illness Claim includes Living Assurance, Early-Payout CriticalCare Plus Claim (EPCC), Critical Care Advantage (CCA), and Cancer Care.

Step 1: Get the following documents

For official proof and verification, we would require you to submit the following documents for the type of claim you are making.

  1. Instruction Page
    (Please use form 1 for Accident claim and form 2 for Golden Protector claim)
  2. Accident Claim Form
  3. For further pre-hospitalisation and post-hospitalisation bills, please submit together with the Submission of further original bills Form 
  4. Clinical Abstract Application
  5. Doctor’s Statement (see Notes below)
    (Please use form 1 for Accident claim and form 2 for Golden Protector claim)
  6. Original medical bills/ receipts/Final Hospital Bills (see Notes (b) below)
  7. Copy of Police Report (if any)
  8. Copy of Medical Certificates
  9. Authorisation Letter (if claimant is authorizing another GEL agent to handle the claim (including collection of cheque)


  1. For claims more than $1,500, the Doctor’s Statement must be completed by the attending doctor and submitted to us. The Doctor’s Statement is furnished at the expense of the claimant.
  2. For claims less than $1,500, the Doctor’s Statement need not be submitted. The Company may waive the medical report if there is sufficient documentary evidence e.g. a Doctor’s Memo or Inpatient Discharge Summary Report to show the cause of hospitalisation/ disability and period of disability.
  3. For the documents mentioned in (3) above, copies of the hospital bills will be accepted for Hospitalisation Allowance/ Benefit claim. Original Final Hospital Bills are required for medical reimbursement claims.

Step 2: Submit documents

Once you have downloaded and completed the relevant claim forms, please submit the claim to us together with the required documents for assessment.
