- What is Care Plus Programme?
Care Plus Programme is a complimentary Disability Programme, which provides a full year (12 months) of disability income benefit for members who signed up for the Programme.
- What are the benefits of Care Plus Programme?
Care Plus Programme provides members S$600 per month, payable up to 60 months, upon the diagnosis of “Severe Disability”, i.e. inability to perform at least three (3) Activities of Daily Living (ADLs). In addition, it also provides a S$3,000 Death benefit should the member dies while receiving the monthly payouts.
- What is the definition of “Severe Disability”?
“Severe Disability” refers to the inability of the Member to perform at least three (3) of the following Activities of Daily Living, even with the aid of special equipment, and which requires significant assistance from another person for the entire activity:
(a) Washing – the ability to wash in the bath or shower (including getting into and out of the bath or shower) or do a sponge / bed bath.
(b) Dressing – the ability to put on, take off, secure and unfasten all garments and, as appropriate, any braces, artificial limbs or other surgical or medical appliances.
(c) Feeding – the ability to feed oneself food after it has been prepared and made available.
(d) Toileting – the ability to use the toilet or manage bowel and bladder function through the use of protective undergarments such as diapers, or surgical appliances if appropriate.
(e) Mobility – the ability to move indoors from room to room on level surfaces.
(f) Transferring – the ability to move from a bed to an upright chair or wheelchair, and vice versa.
- Is membership for the Care Plus Programme free?
Yes, the Care Plus Programme is free to all members for 12 months (from the date of application). After 12 months, the programme will terminate automatically without any further action from you.
- Who is eligible to apply for the Care Plus Programme?
Care Plus Programme is only open to Singapore Citizens and Singapore Permanent Residents who are aged 30 to 40 years old, i.e. Date of Birth must be between 1 January 1980 to 30 September 1990 (both dates inclusive).
- Are there any other factors required to qualify for the Care Plus Programme?
Yes, on top of the eligibility criteria in Q5, you have to clear the Health Declaration and satisfy the BMI requirement.
(i) I do not suffer, nor have I been treated for, nor advised by any medical practioner to seek medical attention for the following conditions: cancer, diabetes, heart attack, heart failure, depression, nervous breakdown, kidney failure, motor neurone disease, multiple sclerosis, arthritis, paralysis, spinal conditions
(ii) In the past 1 year, I do not need assistance of another person or mechanical aids in the performance of my activities of daily living such as washing (bathing), dressing, walking, transferring from bed to chair and maintaining continence, and I have not stopped doing any of the following day to day activities due to my health conditions(s);- housework, preparing meals, opening and sorting mails, using public transport, shopping for groceries or personal needs, participating in social activities.
BMI needs to fall between 16 to 30 (both 16 and 30 not inclusive)
- If I fail the health declaration or BMI requirements, how can I appeal to apply for the Care Plus Programme?
If you fail the health declaration or BMI requirements, you will not be qualified to sign up for the programme. There is strictly no appeal allowed.
- Can I apply for the Care Plus Programme more than once to enjoy more coverage?
No, you can only apply for membership to the Programme once.
- Can I apply for the Care Plus Programme if I am already a member of similar disability income programmes from other insurance companies?
Yes, you can apply for the Care Plus Programme even if you are a member of similar disability income programmes from other insurance companies.
- How do I file a claim under the Care Plus Programme?
To claim under the Care Plus Programme, you will need to:
(a) Inform Great Eastern via Great Eastern’s customer hotline, or in writing at LifePAClaims-SG@greateasternlife.com, that you are suffering from Severe Disability
(b) Complete and submit the necessary claim form(s) as requested by the Company
- What documents do I need to provide to claim under Care Plus Programme?
You will need to provide the following:
(a) Necessary claim form(s) as requested by the Company
(b) Confirmed diagnosis of the Severe Disability, with the Assessor’s Statement completed by the Panel Assessor on your ability to perform at least 3 Activities of Daily Living
(c) Proof of your identity
(d) Any other document necessary to support the claim at the Company’s request
Note: Proof of continuing Severe Disability will need to be furnished from time to time at the Company’s request, via a registered medical practitioner appointed by the Company’s consent
- When will I start getting my monthly payouts if my claim is successful?
The first monthly payment will start after a deferment period of 90 days (“Deferment Period”).
- What does “Deferment Period” mean?
“Deferment Period” refers to the 90-day period from the claim date (inclusive). It will commence from the Claim Date, i.e. the date on which the Assessor from the Appointed Panel has certified you of suffering from Severe Disability.
- My Membership starts on 1 May 2020. Can I claim if I am diagnosed with Severe Disability on 1 June 2021?
No, Care Plus Programme will automatically end after 1 year of membership into the programme. In this case, the membership will end on 30 April 2021, and the claim will be unsuccessful.
- My Membership starts on 1 May 2020. Can I claim if I am assessed by the Panel of Assessor and was diagnosed with Severe Disability on 1 April 2021?
Yes, the claim will be successful if you are assessed to have Severe Disability within the one year Membership period. However, the first payout will only start on 1 July 2021, after the 90 day Deferment Period.
- If my claim is successful, will the Programme pay me S$600 per month continuously for 60 months?
Care Plus Programme will pay up to 60 months, if you continue to suffer from Severe Disability throughout the 60 months from claim.
Note that all monthly payouts will cease upon:
(a) The Member ceasing to suffer from Severe Disability;
(b) The death of the Member;
(c) The Member ceasing to be a Singapore Citizen or Permanent Resident; or
(d) Once the total payouts made by the Company to the Member have reached 60 months;
whichever is earliest
- Are there any exclusions under this Programme?
Yes, no payout will be made in the event of any of the following occurrences:
(a) Pre-existing Disability or a Severe Disability arising from Pre-existing Conditions(s);
(b) Intentionally self-inflicted injury, or attempted suicide whether the Member is sane or insane;
(c) War, whether declared or undeclared, alcoholism, or drug addiction;
(d) Any fraud, non-disclosure, misrepresentation whether intentional, negligent or innocent of a material fact, or a breach of a warranty or a condition on the Member’s part; or
(e) Prohibition under any applicable laws, regulatory policies or other statutory requirements.
