Great Prestige Card Redemption Terms & Conditions
These Redemption Terms & Conditions apply to holders of a valid Great Prestige Card issued by Great Eastern Life Assurance (Malaysia) Berhad ("Great Eastern").
1. A valid Great Prestige Card holder is entitled to redeem the following complimentary privileges during the validity period:
a. One (1) complimentary health screening package from a participating hospital;
b. One (1) complimentary eye screening package from Oasis Eye Specialists Centre; and
c. One (1) complimentary teleconsultation package from BIG Caring Group comprising two (2) Doctor Consultations, one (1) Dietitian Consultation, one (1) Pharmacist Consultation and one (1) Mental Health Counsellor Consultation.
2. Each complimentary privilege may only be redeemed once during the validity period. For the avoidance of doubt, the BIG Caring Group teleconsultation package includes all consultation services specified under Clause 1(c).
3. Complimentary privileges may be redeemed within one (1) year from the policy in-force date, unless otherwise stated. Redemption of any privilege is subject to the relevant policy remaining in force at the time of redemption and successful verification of the Great Prestige Card holder's name, Great Prestige Card serial number (being the TnG card serial number), policy number, NRIC/identification details and such other supporting information as may be reasonably required by Great Eastern or the participating service provider.
4. For hospital health screening packages and eye screening packages, appointments must be arranged via email at prestigeambassador@greateasternlife.com.
5. For the BIG Caring Group teleconsultation package, redemption must be made through the BIG Caring platform using the designated link provided by Great Eastern.
6. All complimentary privileges are subject to availability and the applicable terms and conditions imposed by the respective participating service providers.
7. The participating hospitals, healthcare providers, eye centre and BIG Caring Group are independent third parties and are not agents, representatives or partners of Great Eastern. All services are provided solely by the respective service providers, who shall be solely responsible for the provision, quality and performance of such services.
8. The Great Prestige Card holder acknowledges and agrees that personal information may be collected, used, disclosed and processed by Great Eastern and shared with the participating service providers solely for the purposes of identity verification, appointment arrangement, redemption administration and provision of the complimentary privileges, in accordance with applicable data protection laws and Great Eastern's Privacy Notice, as updated from time to time and available on here.
9. Great Eastern shall not be responsible or liable for the quality, merchantability, fitness for a particular purpose, defect, malfunction or deficiency of any product or service provided by any participating service provider. Great Eastern shall not be liable for any loss, injury, damage, cost or expense arising from or in connection with the use of any product or service provided by any participating service provider, except to the extent such liability cannot be excluded under applicable law.
10. Any medical advice, diagnosis, treatment recommendation or healthcare service provided pursuant to the complimentary privileges shall be the sole responsibility of the relevant participating service provider. Great Eastern does not provide medical advice and makes no representation regarding the suitability of any healthcare service or treatment.
11. Complimentary privileges are not exchangeable for cash, credit or any other benefit, and any unutilised privilege shall lapse automatically upon expiry.
12. Any services, treatments, products or charges incurred beyond the complimentary entitlement shall be solely borne by the Great Prestige Card holder.
13. The Great Prestige Card and all associated privileges are personal to the Great Prestige Card holder and are non-transferable, non-assignable and may not be sold, exchanged or otherwise transferred to any other person.
14. Great Eastern reserves the right to refuse, suspend or revoke any privilege in the event of suspected fraud, abuse, misconduct, misrepresentation or breach of these Redemption Terms & Conditions.
15. Great Eastern reserves the right to amend, vary, substitute, suspend or withdraw any privilege and/or these Redemption Terms & Conditions from time to time, subject to applicable legal and regulatory requirements.
16. These Redemption Terms & Conditions shall be governed by and construed in accordance with the laws of Malaysia.
