We would like to highlight the following for forms marked with (#) as below:
Request For Contractual Changes With Health Declaration (PSF02)
For Investment-linked policy,
- the effective price of the insurance charges of the benefit/rider(s) alteration will be based on the policy's monthlyversary date.
- the expected outcomes of the sustainability test will be provided within 14 days after the fully completed application is received.
- for any alteration(s) which may increase the policy's insurance charges, the policy's Total Investment Value ("TIV") may not be sufficient to sustain your policy up to full contractual term/maturity date.
