Terms and Conditions for Webinar Survey

Eligibility

To receive 200 GREAT Dollars (“Reward”), participants must:

• Be a verified Great Eastern customer using the Great Eastern Rewards app (“App”).
• Complete the survey during "The Health Dialogue" webinars.

Important Details

• Each User can only receive one (1) Reward per survey. Subsequent submissions will be disqualified.
• Eligible participants who are Users of the App will have the Reward credited to their Great Eastern Rewards account. There will be an in-app notification once it’s done, so make sure notifications are turned on.
• To receive the Reward, participants must provide the email address linked to their Great Eastern Rewards account in the survey form.

Reward Credit Timeline

By participating in this survey, you consent to the collection, use and disclosure of your personal data by Great Eastern and its related corporations (collectively, the “Companies”), as well as their respective representatives, agents, authorised service providers and relevant third parties, collecting, using and/or disclosing your personal data for purposes reasonably required by the Companies to administer this Reward and such other purposes as described in Great Eastern’s Privacy Statement which is accessible from Great Eastern’s website.
