Important notes:

  • Applicant(s) is required to complete and submit their LPA Form 1 online and ensure that all Donee(s) and Replacement Donees(s), if any, have accepted their appointments, before attending the certification session.
  • Upon arrival, Applicant(s) is required to present the SMS notification from the Ministry of Social and Family Development (MSF) via "gov.sg" stating that all Donee(s) and Replacement Donees(s) have accepted their appointment and that you may visit a Certificate Issuer to sign your LPA. This SMS is required to obtain your queue number for certification.
