To make the most of your certification session, here's what you'll need to prepare before hand.
1. Submit your LPA Form 1 online
Complete and submit your LPA Form 1 before attending your certification session. Apply for LPA: click here
2. Ensure your Donee(s) have accepted their appointments
All Donee(s) and Replacement Donee(s), if any, must accept their appointments before your session.
3. Present your MSF SMS notification
Upon arrival, please present the SMS notification from MSF via “gov.sg”.This SMS should confirm that all appointments have been accepted and that you may visit a Certificate Issuer to sign your LPA.You will need this SMS to obtain your queue number for certification.

Important notes:
- Applicant(s) is required to complete and submit their LPA Form 1 online and ensure that all Donee(s) and Replacement Donees(s), if any, have accepted their appointments, before attending the certification session.
- Upon arrival, Applicant(s) is required to present the SMS notification from the Ministry of Social and Family Development (MSF) via "gov.sg" stating that all Donee(s) and Replacement Donees(s) have accepted their appointment and that you may visit a Certificate Issuer to sign your LPA. This SMS is required to obtain your queue number for certification.
