Make a critical illness claim
Before making a claim
If you're unsure on your exact diagnosis, consult your doctor and seek their opinion with your policy and its list of definitions for each major illness.
If your doctor believes your conditions fulfil any one of the major illnesses, request your doctor for a Doctor's Statement of that major illness.
Refer to the relevant medical condition below to find out how to get a Doctor's Statement.

How to make a critical illness claim
The forms and supporting documents needed are different for each diagnosis. Start by selecting the relevant diagnosis for your claim.
1. Download forms

2. Include supporting documents
In addition to the forms and documents in the previous step, we'll also require you to submit the following supporting documents from your doctor:
- Histopathology or biopsy reports for cancer
- All available laboratory and test results as specified in the Doctor's Statement for your claim type

How to apply for a Doctor's Statement
The process is slightly different depending on the type of hospital at which your diagnosis was made. Note that the Doctor's Statement can only be completed by the doctor who certified your condition.
For restructured hospitals (public hospitals)
a. Download and print the Doctor's Statement template and bring it along with you to the hospital.
b. Apply through the hospital's Medical Report Office.
c. Make payment.
d. Your application may take approximately 6 to 8 weeks.
For private hospitals
a. Download and print the Doctor's Statement template and bring it along with you to the hospital.
b. Pass the Doctor's Statement to your attending doctor to complete during your visit.
c. Make payment.
d. Your Doctor's Statement will be completed during your visit.

New and reinstated policies
There is a 90-day waiting period for the following major illnesses and surgeries (i.e. the major illness or the illness for which surgery is required will be covered only 90 days after the date of issue of the policy/rider or reinstatement of the policy/rider):
- Cancer and major cancers
- Heart attack
- Angioplasty and other invasive treatments for the coronary artery
- Coronary artery by-pass surgery
Questions and Answers
The life insurance benefits provide a worldwide cover. However, the insured person’s condition must be certified by a qualified physician practising Western medicine. We reserve the right to ask the insured person to be reviewed by an appointed physician.
If a special endorsement (Endorsement No. 100) is included in the insured person's policy, e.g. for foreigners and PRs, the insured person is required to be diagnosed in Singapore, Malaysia or Brunei.
However, we will still consider the medical evidence if the illness is diagnosed in other countries. If the medical evidence is sufficient, we may choose to pay the claim on a goodwill basis without admitting liability. Otherwise, we reserve the right to ask the insured person to be reviewed by our appointed physician.
