Make a female-related illness claim

Follow these steps to make a claim for the Life Women Enhanced Benefits Plan.

Step 1: Prepare the required documents

Download these documents:
  1. Claimant's Statement
    Complete this form for us to find out more details.
  2. Clinical Abstract Application
    This form provides us with your consent to attain your medical information from the hospital on your behalf.
  3. Doctor’s Statement
    Learn more about completing the document below.
  4. Authorisation Letter
    Authorise your Financial Representative to handle the claim including the collection of cheque on your behalf.
(i) For claims on the following two illnesses:
  1. Hospitalisation due to Complications of Pregnancy or Childbirth
  2. Incubation of New-Born Baby for more than 5 Days
(ii) For all other female-related illnesses:

Step 2: Attach supporting documents

  • All available laboratory and test results.
  • Carcinoma in situ of the Breast
  • Carcinoma in situ of the Cervix Uteri
  • SLE with Lupus Nephritis
  • Hysterectomy required as a result of Cancer
  • Ectopic Pregnancy
  • Reconstructive Surgery of Facial Disfigurement
  • Hospitalisation due to Complications of Pregnancy or Childbirth
  • Incubation of New-Born Baby for more than 5 Days
  • Down's Syndrome
  • Spina Bifida
  • Tetralogy of Fallot
  • Transposition of the Great Vessels
  • Disseminated Intravascular Coagulation

Depending on the type of hospital, the process will be different and applicable after you have been diagnosed. Download and print the relevant Doctor's Statement template (as above) and bring it along with you to the hospital/clinic. The Doctor Statement is to be completed by the doctor who diagnosed you. You will be required to pay a fee for the application. 

For restructured hospital (public hospital), apply through the hospital's Medical Report Office (MRO). This may take approximately 6-8 weeks.

For private hospital, pass the doctor's statement to your attending doctor to complete during your visit.
