We are sorry to hear about your illness. You can make a claim for the following female-related illnesses:
- Carcinoma in situ of the Breast
- Carcinoma in situ of the Cervix Uteri
- Disseminated Intravascular Coagulation
- Down's Syndrome
- Ectopic Pregnancy
- Hospitalisation due to Complications of Pregnancy or Childbirth
- Hysterectomy required as a result of Cancer
- Incubation of New-Born Baby for more than 5 Days
- Reconstructive Surgery of Facial Disfigurement
- SLE with Lupus Nephritis
- Spina Bifida
- Tetralogy of Fallot
- Transposition of the Great Vessels
How to make a female-related critical illness claim
1. Download documents
You will need to submit the following documents for your claim.

How to apply for a Doctor's Statement
The process is slightly different depending on the type of hospital at which your diagnosis was made. Note that the Doctor's Statement can only be completed by the doctor who certified your condition.
For restructured hospitals (public hospitals)
a. Download and print the Doctor's Statement template and bring it along with you to the hospital.
b. Apply through the hospital's Medical Report Office.
c. Make payment.
d. Your application may take approximately 6 to 8 weeks.
For private hospitals
a. Download and print the Doctor's Statement template and bring it along with you to the hospital.
b. Pass the Doctor's Statement to your attending doctor to complete during your visit.
c. Make payment.
d. Your Doctor's Statement will be completed during your visit.
2. Include supporting documents
In addition to the forms and documents in the previous step, we'll also require you to submit other supporting documents you may have for your condition, such as laboratory and test results.

Questions and Answers
The life insurance plan provides a worldwide cover. However, the life assured’s condition must be certified by a qualified physician practising Western medicine.
We reserve the right to ask the life assured to be reviewed by an appointed physician. If a special endorsement (Endorsement No. 100) is included in the life assured's policy, e.g. for foreigners and PRs, the life assured is required to be diagnosed in Singapore, Malaysia or Brunei. However, we will still consider the medical evidence if the illness is diagnosed in other countries.
If the medical evidence is sufficient, we may choose to pay the claim on a goodwill basis without admitting liability. Otherwise, we reserve the right to ask the life assured to be reviewed by an appointed physician.
