Make an injury, hospitalisation, or death claim
If your maid or foreign worker suffers any accidental bodily injury or illnesses that require hospitalisation and surgical care, you can contact your Financial Representative for assistance, or call our General Claim hotline at +65 6248 2638 for advice.

How to make claim for injury, hospitalisation, or death to your maid or foreign worker
1. Make a police report
You'll need to make a police report if the injury is serious or fatal.
2. Get a medical report
A medical report is required from your maid or foreign worker's attending doctor stating the following:
- Date of onset
- Cause of injury or illness
- Nature of injury of illness
Note that all claimants are required to bear the cost of getting the medical report.

To submit your claim for injury to your maid or foreign worker, we'll require you to complete and submit one of the following relevant forms:
