Complimentary Home-based Disability Assessments Campaign (The “Campaign”) Terms and Conditions

1.      The Campaign is organised by The Great Eastern Life Assurance Company Limited (“Great Eastern”) for new and existing policyholders of at least 1 of the following eligible plans (“GREAT CareShield”) during the Campaign Period:-

  • GREAT CareShield Enhanced
  • GREAT CareShield Advantage
  • GREAT CareShield Supreme

collectively referred to as “GCS Policyholders”

2.     The Campaign will run from 01 July 2026 till 31 December 2027, both dates inclusive (“Campaign Period”). Great Eastern reserves the right to amend the dates without prior notice.

3.     During the Campaign Period, GCS Policyholder will be entitled to a complimentary access (administered by Care@Homes (“Provider”)) comprising of the following benefits:-

                 i.          Up to two (2) complimentary home-based Severe Disability Assessments (SDA) conducted by Ministry of Health (MOH) accredited assessor;

               ii.          Up to two (2) complimentary home-based Functional Assessment Reports (FAR) conducted by a Singapore-registered doctor; and/or

              iii.          Up to 20% discount off products and services offered by Provider.

hereby collectively referred to as “Complimentary Access”.

4.     Complimentary Access is non-transferable and cannot be exchanged for cash or other goods or services.

5.     GCS Policyholders will receive a notification of their Complimentary Access redemption details during the following periods via push notification from the Great Eastern Rewards mobile application, Email and/or any other communications method to which you have given marketing consent to Great Eastern:

6.     To redeem the Complimentary Access, each eligible GCS Policyholder (each, an “Applicant”; together, the “Applicants”) must complete the following steps:

                 i.          Log in to their Great Eastern Rewards mobile application using their SingPass or GreatID;

               ii.          Retrieve the code (“Promotion Code”);

              iii.          Go over to Provider dedicated website (https://careathomes.com.sg/pages/gcs-benefit);

              iv.          Activate the Complimentary Access by providing the required details;

               v.          Apply the Promotion Code at the payment checkout page.

7.     Upon successful activation of the Complimentary Access, Applicants will receive a notification from the Provider via email.

8.     If Great Eastern subsequently determines that a person is in fact not eligible for the Complimentary Access, for any reason whatsoever, Great Eastern may at its discretion, disqualify the GCS Policyholder and cancel the Complimentary Access without prior notice.

Personal Data Protection

9.     By participating in the Campaign, each GCS Policyholder agrees and consents to Great Eastern and its related corporations (collectively, the “Companies”), as well as their respective representatives, agents, authorised service providers and relevant third parties (the Companies and all the other foregoing parties, collectively, “Great Eastern Persons”), collecting, using and/or disclosing each GCS Policyholder’s personal data, for purposes reasonably required by the Companies to administer the Campaign, process the Complimentary Access, and such other purposes as described in Great Eastern’s Privacy Statement which is accessible from Great Eastern’s website, which each GCS Policyholder confirms he/she has read and understood.

10.   Each GCS Policyholder’s consents herein supplement but do not supersede nor replace any other consents they may have previously provided to Great Eastern Persons, and their consents herein are cumulative and additional to any rights which any of the Great Eastern Persons may have to collect, use, and/or disclose of that applicant’s Personal Data, with or without that GCS Policyholder’s consent, to the extent permitted under applicable law.

General

11.    The decision of Great Eastern on all matters relating to the Campaign and Complimentary Access shall be final and binding on all participating GCS Policyholders / Applicants.

12.   Great Eastern may vary these terms and conditions or suspend or terminate the Campaign/Complimentary Access without any notice or liability to any party.

13.   Great Eastern may replace, withdraw or add to the Complimentary Access at any time without notice or liability.

14.   In the event of any inconsistency between these terms and conditions and any advertising, publicity, brochure, marketing or other materials relating to or in connection with this Campaign, these terms and conditions shall prevail.

15.   Great Eastern will not be responsible for any injuries, loss, claim or damage suffered or incurred in connection with the Campaign, the Complimentary Access and/or any notices, letters or correspondence lost, stolen or misdirected in the postal system or telecommunication system, and the consequences arising from the non-receipt of such communication.

16.   By participating in this Campaign, the GCS Policyholders/Applicants agree to release and hold Great Eastern harmless from any and all liability whatsoever for any injuries, losses or damages of any kind to any person or property arising from or in connection with, either directly or indirectly:

a)     the awarding, acceptance, receipt, possession, use or misuse of Complimentary or parts thereof awarded pursuant to the Campaign; and

b)    the participation in the Campaign or any Complimentary Access-related activities.

17.   Notwithstanding anything herein, Great Eastern has the absolute discretion to determine the eligibility of any person to participate in the Campaign.

18.  The terms and conditions of this Campaign/Complimentary Access shall be governed by and interpreted in accordance with Singapore law. The courts of Singapore shall have exclusive jurisdiction over any disputes arising from the terms and conditions, including the validity and enforceability thereof.
