If you wish to authorise a Great Eastern Financial Representative (other than your designated Financial Representative for this policy) to handle the claim, complete the authorisation letter and submit it together with the other forms and documents.

How to make a personal accident claim

In addition to the form in the previous step, we'll also require you to submit the following documents:

For personal accident:

  • Medical report from attending doctor

For death:

  • A copy of the death certificate
  • Autopsy report, coroner’s findings, etc.
  • Copy of police report (if any)
  • Letter of Administration, Grant of Probate, or will (only for claims exceeding SGD150,000)
  • Proof of relationship between the deceased and claimant

For permanent or temporary disablement:

  • Medical report from attending doctor
  • Copy of medical certificates

For medical expenses:

  • Original medical bills and receipts
  • Inpatient discharge summary in the case of hospitalisation
  • Medical report from attending doctor

Note: Copies of the hospital bills will be accepted for hospitalisation allowance or benefit claim. Original final hospital bills will be required for medical reimbursement claims.

1. Download documents

You will need to submit the following documents for your claim.

Claimant's Statement To be completed with details of your claim
Clinical Abstract Application To allow us to access your medical information
Doctor's Statement Required only if your claim is more than SGD1,500
 
For claims less than SGD1,500, the Doctor’s Statement is not needed if there is sufficient documentary evidence, e.g. a Doctor’s Memo or Inpatient Discharge Summary Report to show the cause of hospitalisation, or disability and period of disability.
 
For claims under Golden Protector:

How to apply for a Doctor's Statement
 
The process is slightly different depending on the type of hospital at which your diagnosis was made. Note that the Doctor's Statement can only be completed by the doctor who certified your condition.
 
For restructured hospitals (public hospitals)
 
a. Download and print the Doctor's Statement template and bring it along with you to the hospital.
 
b. Apply through the hospital's Medical Report Office.
 
c. Make payment.
 
d. Your application may take approximately 6 to 8 weeks.
 
For private hospitals and specialist clinics
 
a. Download and print the Doctor's Statement template and bring it along with you to the hospital.
 
b. Pass the Doctor's Statement to your attending doctor to complete during your visit.
 
c. Make payment.
 
d. Your Doctor's Statement will be completed during your visit.
 
2. Include supporting documents
 
In addition to the forms and documents in the previous step, we'll also require you to submit the following documents:
 
  • Original final hospital and medical bills and receipts
  • Copy of laboratory reports – X-ray report, CT report, MRI report, etc.
  • Copy of medical certificates for total & partial disability claim
  • Copy of policy report or media report about the accident (if any)

Note: Copies of the hospital bills will be accepted for hospitalisation allowance or benefit claim. Original final hospital bills will be required for medical reimbursement claims.
