What type of claim would you like to submit?


Step 1: Prepare the following documents

To verify your data, you need to submit the following documents according to the type of claim you are submitting. 

For Death Claims:

  1. Native police
  2. Death Claim Form filled out correctly and completely and signed by the Heirs
  3. Power of Attorney for Provision of Medical Record Information (second page of Claim Form)
  4. Doctor's Certificate Form for Death Claim
  5. Photocopy of the Insured's Identity Card
  6. Photocopy of Family Card
  7. Original/legalized photocopy of certificate of heirship authorized by the local government
  8. Original/legalized photocopy of Death Certificate from Civil Service or Hospital
  9. Original/legalized photocopy of Burial or Cremation Certificate
  10. Original/legalized photocopy of the Accident Report that has been legalized by the local police (if the death was caused by an accident)
  11. Death Certificate from the Embassy if you die abroad
  12. Other documents are adjusted to the description below:

Step 2: Submit documents

Once you have downloaded and completed the claim form, submit your claim along with supporting documents for verification directly to the Customer Service Center at our head office.

Check Your Claim Status

If a complaint occurs regarding the purchase of a product/and use of a service, the Prospective Policy Holder/Customer can contact  the Customer Contact Center through the methods and mechanisms determined and notified by the Company. 

The Company is obliged to resolve complaints no later than within 20 (twenty) working days and the Company may extend the time limit for resolving complaints up to the following 20 (twenty) working days by notifying the Policyholder in accordance with applicable laws and regulations.
